Toadstool Cafe is Toad's restaurant, and it commits completely. Burgers shaped like characters, a mushroom soup served in a bread bowl, a caprese salad dressed as a Piranha Plant, a tiramisu built as a question block. Every plate is a piece of set dressing, and the kitchen visible behind the counter is staffed by Toads working over the food.
What surprises people is that it tastes fine. This could easily have been a photograph you pay twenty dollars for, and instead it is a competently cooked meal that happens to look ridiculous. The burger is a real burger; the soup is a real soup.
It is the single best themed meal at Epic Universe and the one children will remember. It is also inside a world that may be gated by a queue system, which makes it the hardest meal in the park to schedule.

With children, yes — it is the meal they will remember from the trip and the food is genuinely decent rather than a photograph you pay for. Without children it is an expensive novelty and Café L'air De La Sirène is the better lunch.
Yes, through the Universal app, and you should order the moment you enter SUPER NINTENDO WORLD. It is the busiest restaurant in the park and the pickup windows fill early.
Yes, it sits inside the world, so if entry is being managed by a virtual queue or timed entry that day, your meal is gated by the same system. Plan the meal around the entry rather than the other way round.
